Shine a flashlight into each eye noting the direct as well as the consensual constriction of the pupils. The swinging flashlight test is used to test for a relative afferent pupillary defect or a Marcus Gunn pupil. Swinging the flashlight back and forth between the two eyes identifies if one pupil has less light perception than the other. Shine the flashlight at one eye noting the size of both pupils. Then swing the flashlight to the other eye. If both pupils now dilate then that eye has perceived less light stimulus (a defect in the sensory or afferent pathway) than the opposite eye.

But you are only observing this bilateral response in the eye with the light in it !! Makes sense once explained, but not exactly intuitive and has stumped many medical student and resident in clinical exams - These APD cases are routinely brought in each year for clinical exams !! ;-)

He underwent Laparoscopic Near Total Gastrectomy with D2 Lymph Nodal Clearance. Technique involves two gauze pieces to lift the body of the stomach off the pancreas for lymph nodal dissection. Procedure involves adhesiolysis of the dense adhesions of previous surgery stapler division of the afferent loop jejuno jejunal anastamosis between the afferent and efferent J loops division of the efferent jejunum at its attachment with stomach completion of gastrectomy as high as possible with D2 clearance of LN and gastro jejunal anastamosis- side to side with endo GIA over calibration tube. Path revealed poorly diff carcinoma of stomach with 9 LN involved with tumor. Patient was discharged in 6 days time and the barium study during the first opd review revealed a small remnant stomach with an adequate anastamosis.
