In 1994, she underwent open gastric bypass surgery but never achieved the desired weight loss. Esophagogastro-duodenoscopy showed a large pouch. The patient was taken to the OR for laparoscopic revision of the bypass. The stomach was mobilized, and by using green load EndoGIA, the stomach was divided just below and above the previous staple lines. Fundus was then divided and resected. The jejunum was divided from the gastrojejunostomy by using a blue load EndoGIA. The gastrojejunostomy was reconstructed in 2 layers over a 34F bougie. On POD 1, UGS showed a functional small pouch without evidence of leakage. At 6-month follow-up, the patient had lost 81 pounds. Laparoscopic revision of open Roux-En-Y gastric bypass surgery is safe and feasible, even in patients who have had a previous open weight loss procedure. He survived 18 months with gemcitabine chemotherapy. One month after insertion of a metal stent and surgical gastrojejunostomy, the tumor had rapidly ingrown and overgrown the stent, obscuring it completely in the duodenum. He had severe sepsis and cardiac arrest from ventricular fibrillation due to acidosis, hypokalemia and hypotension. He was resuscitated and urgent ERCP was performed to insert a covered metal stent within the lumen and through the mesh of the previous uncovered metal stent. Technically the challenges were 4 fold: (1) intubated patient on assisted ventilation in the supine position, (2) accessing the area after a gastrojejunostomy in the "long" position, (3) locating and cannulating the metal stent with fluoroscopy guidance, using trial and error, with the papilla in the unfavourable 6 o'clock position, (4) insertion, deployment of the covered metal stent which has a stiff delivery system and overcoming U-shape knuckling of the system in the duodenum - performed under challenging conditions of limited maneuvrability and visibility. Recurrent cholangitis remains the Achilles heel of endoscopic biliary drainage since its inception almost 20 years ago. ERCP biliary stenting is a good treatment for patients with a "bad prognosis" and short survival. It is not such a good treatment for patients with a "good prognosis" who survive more than 6 months because ...

Causes of gastric outlet obstruction is either benign cause ie chronic duodenal ulcer, benign growth at antrum, any growth compressing from outside causing obstruction, malignant cause like cancer stomach. Symptoms of gastric outlet obstruction- patient complains of vomiting ,projectile containing undigested food particle devoid of bile, history of peptic ulcer and weight loss. Signs one should find- dehydration, wasting, visible peristalsis, succession flash, electrolyte imbalance, hypochloremic, hypokalemic, hyponatremic metabolic alkalosis. Investigations of gastric outlet obstruction- upper GI endoscopy give a confirmatory diagnosis and biopsy can be taken from suspected sites. X-ray abdomen, Ba --meal follow through can also give diagnosis of gastric outlet obstruction. CT scan helps in detecting other causes of GOO not arising from stomach Management -- partial gastric outlet obstruction patients can be tried with medical management with H2 blockers. Correction of electrolyte imbalance. Surgical treatment includes-vagotomy & antrectomy, vagotomy & pyloroplasty, truncal vagotomy and gastrojejunostomy. Vagotomy, antrectomy with gastrojejunostomy gives best result. Balloon dilatation can be tried but have recurrence rate. At expert centres laparoscopic vagotomy with gastrojejunostomy gives promising results with minimal morbidity.
