  • Severe Tissue Injury with IV Promethazine The Institute for Safe Medication Practices recently warned that administering promethazine parenterally may cause more severe tissue injuries than previously recognized. Promethazine, which is commonly sold under the trade name Phenergan, has many uses including as an antihistamine, a sedative and an antiemetic. ISMP points that out promethazine is a known vesicant which is highly caustic to the intima of blood vessels and surrounding tissue. Deep intramuscular injection into a large muscle is the preferred parenteral route of administration, and is least likely to cause damage. But promethazine is also given by slow IV push in many hospitals. The IV route, along with inadvertent intra-arterial or subcutaneous administration is most likely to result in significant complications, ranging from burning and pain to paralysis, tissue necrosis, and gangrene. Sometimes surgical intervention such as fasciotomy, skin graft or even amputation is needed. ISMP describes several recent cases where severe tissue damage occurred following IV administration of promethazine. For example, in one case a 19-year-old woman who went to the emergency department with flu-like symptoms was given Phenergan IV. During the injection, she cried out in pain, and after the injection she told the nurse that her arm was still in significant pain and that she felt "something was wrong." The nurse reassured her and then left the room. The patient's arm and fingers became purple and blotchy and then over ...

  • Microsurgery Tutorials. Bonus: Heterotopic Heart Transplant. BN Rat A series of tutorials on microsurgery, starting from the basics working up to more advanced techniques, uploaded in random order. All videos: Copyright 2012 Victor Volovici (DrW). Carl Zeiss Opmi Pico Microscope, no colour filter applied, magnification 10x- 50x. Heterotopic heart transplantation model, with the Pulmonary Artery connected to the IVC of the recipient and the Aorta connected to the Abdominal Aorta of the recipient. The Donor heart is harvested after systemic heparinization 10 UI/10 gram, as follows: the IVC and SVC are ligated, the aorta an pulmonary artery cut before the emergence of the brachiocephalic trunk and the split into left and right arteries respectively. The pulmonary veins are then ligated and the heart is removed and placed in 4 degrees Celsius cold Custodiol organ preservation solution. The recipient is apporached via a large xyphopubic incision. The aorta and IVC are dissected, incised with a 30G needle after being ligated superiorly(just under the renal artery and vein) and inferiorly(just above the iliac vessels) with 6-0 Monolon. Terminal-lateral anastomoses are performed on both the aorta and the pulmonary trunk with great finesse so as not to damage the very delicate tissue. We advise working with the same care as in lesson 3, petal suture. After anastomoses are performed, the distal and proximal ligatures respectively are opened and the heart resumes normal function. The anastomosis time should be under 30 minutes. Do ...
