Guillain-Barré syndrome is a disorder in which the body's immune system attacks part of the peripheral nervous system. The first symptoms of this disorder include varying degrees of weakness or tingling sensations in the legs. In many instances, the weakness and abnormal sensations spread to the arms and upper body. These symptoms can increase in intensity until the muscles cannot be used at all and the patient is almost totally paralyzed. In these cases, the disorder is life-threatening and is considered a medical emergency. The patient is often put on a respirator to assist with breathing. Most patients, however, recover from even the most severe cases of Guillain-Barré syndrome, although some continue to have some degree of weakness. Guillain-Barré syndrome is rare. Usually Guillain-Barré occurs a few days or weeks after the patient has had symptoms of a respiratory or gastrointestinal viral infection. Occasionally, surgery or vaccinations will trigger the syndrome. The disorder can develop over the course of hours or days, or it may take up to 3 to 4 weeks.   • Antibodies "Antibodies of the immunoglobulin G class are Y-shaped glycoproteins that circulate in the bloodstream. They bind to and inactivate foreign molecules—the antigens—and mark them for destruction. Each IgG molecule consists of two light chains and two heavy chains. The heavy chains have carbohydrates attached. The regions of the antibody that bind to antigens are located at the very tips of the two arms. Each arm of the antibody is composed of four domains. Two are called the variable domains, contributed by the heavy and light chains, and hence called VH and VL. The variable domains are attached to two constant domains, again one each from the heavy and light chains, and hence called CH and CL. Variable and constant domains share a similar structure, called the Ig fold. Each domain consists of a pair of beta sheets, one with three strands and one with five. A single covalent disulfide bridge holds the two sheets together, which results in a rigid and very stable domain. The tertiary structure of a protein, in this case an antibody IgG, allows it to bind to only specific molecules, its respective antigen.   • Rosie gets an Intragam - Visit I had developed a condition called hypogammaglobulinemia. My Oncologist suggested this may have been a possible response from my earlier chemotherapy treatment. What it meant was my immune system had been compromised and my bodies ability to fight off infections was very low. I was experiencing many re current infections. Because of this I was offered to be treated with a immunoglobulin infusion called Intragam. INTRAGAM P is a preparation of normal human immunoglobulin, which is usually found in the body. Immunoglobulins are proteins produced by plasma cells (white blood cells) to aid in fighting infections. Intragam P is used to treat patients who need replacement of antibodies which form part of our immune system and can provide protection against some infections. The blood is collected from voluntary donors and prepared for intravenous use, infusion into a vein. In total, this procedure can take up to three to four hours. Vital signs are monitored regularly throughout the infusion, in line with hospital's policy. There can be adverse reactions, but I was always fine. Each time I have this therapy I would develop over the next few days a few minor reactions such as bone pain, headaches, abdomen (period pain), tiredness, reduced smell and taste and itchy skin. A blood sample must be drawn. The patient's red blood cells may have antibodies (and other complement system factors) on the surface of some of the red blood cells (abbreviated as RBCs in this animation). The blood sample is washed to remove the patient's plasma while leaving the red blood cells intact with their attached antibodies. These red blood cells are incubated with anti-human globulin containing both anti-IgG and anti-complement. The anti-human globulin binds to the patient's antibody, coating the red blood cells, resulting in agglutination. Results The implantation rate (number of gestational sacs per embryo transferred, with an average of two embryos transferred by cycle) was 59% (50/85), 47% (21/45), 31% (4/13) and 0% (0/9) for groups I, II, III and IV respectively. The clinical pregnancy rate (fetal heart activity per IVF cycle started) was 80% (33/41), 57% (13/23), 50% (3/6) and 0% (0/5) and the live birth rate was 73% (30/41), 52% (12/23), 50% (3/6) and 0% (0/5) respectively. There was a significant improvement in implantation, clinical pregnancy and live birth rates for group I versus group IV (P = 0.0007, 0.0009, and 0.003, respectively) and for group II versus group IV (P = 0.009, 0.04 and 0.05, respectively).
