Epilepsy is characterized by repeated seizures caused by recurrent, abnormal, and excessive synchronous discharges from cerebral neurons. This basic pathophysiological understanding of epileptic seizures was deduced by the 19th-century physician John Hughlings Jackson based solely on clinical observations. His contributions to the field of epilepsy continue to be recognized by modern medical sciences. Jackson, like many before and after him, recognized that epileptic seizures take many forms and have many causes. The first known description of epilepsy appears in the Hippocratic text of 350 BC, and epilepsy has been called the "Hippocratic disease." The pancreatic islet is a unique micro-organ composed of several hormone secreting endocrine cells such as beta-cells (insulin), alpha-cells (glucagon), and delta-cells (somatostatin) that are embedded in the exocrine tissues and comprise 1-2% of the entire pancreas. There is a close correlation between body and pancreas weight. Total beta-cell mass also increases proportionately to compensate for the demand for insulin in the body. What escapes this proportionate expansion is the size distribution of islets. Large animals such as humans share similar islet size distributions with mice, suggesting that this micro-organ has a certain size limit to be functional. The inability of large animal pancreata to generate proportionately larger islets is compensated for by an increase in the number of islets and by an increase in the proportion of larger islets in their overall islet size distribution.   • Mechanismy hloubkové mozkové stimulace (Mechanism of deep Brain Stimulation) Katedra kybernetiky FEL ČVUT nit.felk.cvut.cz Hluboká mozková stimulace (Deep Brain Stimulation - DBS) subthalmického jádra (STN) je důležitou metodou léčby Parkinsonovy nemoci. Ačkoliv o příznivých terapeutických účincích STN DBS na hybnost není pochyb, naše znalosti o patofyziologických mechanismech neurostimulace jsou nedostatečné. STN jádro přitom není zapojeno jen do motorických okruhů. V předkládáném projektu se kromě patofyziologických aspektů řízení hybnosti u člověka budeme věnovat účinkům STN DBS na zrakové funkce a na percepci podnětů s afektivním nábojem. Pomocí funkční magnetické rezonance (fMRI) chceme vizualizovat kortikální a subkortikálních oblastí, které jsou díky STN DBS ovlivněny. Vzhledem k rozdílným účinkům farmakologické a neurostimulační léčby předpokládáme, že dokážeme prokázat odlišnosti mechanismu STN DBS od centrálních účinků opaminergní léčby. Pomocí peroperační mikroregistrace jednotkové aktivity chceme nalézt důkaz o existenci subthalamických neuronů zapojených do afektivních okruhů. ředpokládáme, že naše experimenty přispějí k pochopení mechanismů neurostimulace, ale ik obecnému poznání role bazálních ganglií mozku člověka.
