  • Live Surgery for Resection of Temporal Lobe Epilepsy: (1 of 3) (VIEWER DISCRETION ADVISED) Informational video produced by the Good Samaritan Hospital & Medical Center in the 1980s about temporal lobe epilepsy surgery. Includes graphic live footage, so viewer discretion advised - do not watch if you get queasy over medical procedures. According to a study by Jutlia, et al. (J Neurol Neurosurg Psychiatry 2002;73(5):486494), 46% of patients with unilateral temporal lobe epilepsy became seizure free, 10% had only postoperative auras, and 15% had rare seizures on follow-up for 5.4 (SD 2.6) years for 140 consecutive adult patients operated on for drug-resistant temporal lobe epilepsy between 1975-1999 at Zurich University Hospital. With minimum post-operative follow-up of greater than 1 year, 66.9% of patients free from disabling seizures (Engel class I) compared with 57.1% who were completely seizure and aura free (ILAE class 1). 70% of the patients received reductions in their antiepileptic drug treatment at the time of the last available follow-up review. Complications related to the surgical procedures were rare.
  • Shoulder Surgery Rehabilitation Exercises | Part 9 | Shoulder Rehab Guide | Greater Denver Area Dr. Peter J. Millett is an orthopedic shoulder specialist who is expert in shoulder surgery and arthroscopic shoulder repair. He is an innovator in new surgery techniques and in rehabilitation. He offers this video to help patients who have recently undergone shoulder surgery; it shows specific exercises, strengthening moves and post-operative rehab techniques to gain better active range of motion. The successful result of any shoulder surgery is dependent upon the surgical skills from the orthopedic surgeon as well as the rehabilitation protocol. The postoperative rehabilitation begins with close communication between the surgeon, his medical staff and the physical therapy team to discuss the patient's medical history, review surgical findings and procedures, and establish the rehabilitation guidelines. This communication continues throughout the recovery process and is an essential component in ensuring a successful outcome. Each individual has his or her own timeframe of progression and treatment response and deserves a made-fit rehabilitation in which healing time lines and specific goals are included. • Sage 2% Chlorhexidine Gluconate (CHG) Cloths Sage® 2% Chlorhexidine Gluconate Cloths Establishing a comprehensive program to help protect patients from Surgical Site Infections, or SSIs is imperative. According to the CDC and the Institute for Healthcare Improvement: 38 percent of all nosocomial infections in surgical patients are Surgical Site Infections1 2.6 to 5 percent of surgical procedures result in these infections1,2 Postoperative wound infections can increase length of stay in hospital by an average of 7 to 10 days1 And, the average cost is greater than 25000 dollars per SSI3 Preoperative skin preparation is an important part of any infection prevention program. Reducing microorganisms on the skin will address one of the risk factors associated with these complications. Most postoperative wound infections are caused by endogenous bacteria. Several common pathogens are likely to reside on the skin, including: Staph aureus, Coag-negative Staph, E. coli and MRSA. Greatly reducing the number of microorganisms is the goal of any preoperative skin prep product. Sage has developed an entirely new chlorhexidine gluconate-or CHG-formula, and put it in a unique disposable cloth to make application easier. Prepping with the non-abrasive, textured cloth promotes even distribution of CHG on the skin. Now, skin antisepsis can be achieved in one-step. Chlorhexidine is a unique cationic molecule with a vital characteristic of persistent antimicrobial activity. Once on the skin, CHG binds to the bacterial surface ...

  • Post-Operative Care Optimized Video Transcript: Emily has recently undergone surgery, and has just returned home from the hospital. Most patients like Emily are discharged from the hospital with written instructions to guide their recovery and an overwhelming amount of materials—this can make identifying and solving post-operative issues confusing and unnecessarily time-consuming. But this won't be the case for Emily, because Emily has RALPH. RALPH is a recovery application for the home, designed specifically for patients recovering from surgery. Its modular program is based around an interactive avatar that guides, educates, and advises families on their post-operative recovery. RALPH has an avatar that represents Emily. When Emily takes her medication and follows her post-operative regimen, the Avatar becomes customizable, and Emily can play with it. But if Emily fails to follow her recovery instructions, RALPH will not let her customize her Avatar. While the Avatar motivates Emily to follow her regimen, it also interfaces with medication and catheterization tracking and reminders, data collection forms, and educational materials. This is how RALPH records Emily's recovery. When Emily experiences abdominal pain, her mother panics. While this could be an indicator of a minor and easily solvable problem, it could also be a sign of a major complication. Using RALPH's step-by-step diagnosis screen, Emily's mother is able to determine that her daughter's catheter is plugged. RALPH instructs Emily's ...

  • Retinal Detachment - Post Operative Recovery - Application of eye drops Emergency eye surgery + 4 days. Recuperating from surgery to attempt to fix a detached retina in my right eye. I started seeing an increase in the number of floaters and flashing lights starting on Sunday last, which then gradually faded away during Memorial day and was replaced on Tuesday with a persistent shadow in my right peripheral vision at approximately the 3 o'clock position. I knew that was bad and so finally saw my awesome optometrist, Dr. Lebovitz, who was a past president of the western pa ophthalmology association. I had an Optomap digital retinal examination and then aa conventional slit lamp exam which confirmed a horseshoe tear in my retina which required immediate corrective surgery. My surgery was performed at UPMC Mercy by Dr. Robert L. Bergren from Retina Vitreous Consultants who performed three procedures in an attempt to save my vision. He did laser photocoagulation to stop the bleeding, he then placed a sclera buckle around my eyeball to put pressure on my retina to reattach to it's support tissue, and then finally, he performed a pneumatic retinopexy which means he injects a gas bubble into my eye and I have to lay on my side in a face down position for a week or two in order to keep the bubble in the correct place and provide another mechanism whereby my retina will reattach. The goal, of course is to have all of this done before the light sensitive layers of my retina die from lack of nourishment and oxygen. Please keep me in your thoughts and ...

  • Khaw advances in trabeculectomy Glaucoma filtration surgery was previously performed when patients had uncontrolled intraocular pressures on maximally tolerated medical treatment, or after failed laser trabeculoplasty. The main reasons for delaying surgery were the risk of post-operative complications associated with standard trabeculectomy procedures and high failure rates for operations in certain sub-groups of glaucoma patients. Technical modifications to the trabeculectomy procedure including adjustable suture techniques combined with the use and techniques of applying powerful antimetabolites now enable the surgeon to have much greater control of both the operation and post-operative scarring. With Professor Peng Tee Khaw, Duckworth and Kent has a complete set of instruments for a system of glaucoma surgery known as the Safer Surgery System that has evolved at Moorfields Eye Hospital in London. The safer surgery system's development is based on a need to improve the consistency. The use of strong antimetabolites such as Mitomycin-C (MMC) during trabeculectomy increases the risk of potential complications, including hypotony, visual loss, uncomfortable blebs and leaking blebs that may lead to endophthalmitis. The Safer Surgery System is designed to preserve visual acuity by minimizing hypotony and bleb-related complications while achieving a desirable postoperative intraocular pressure. Mitomycin-C is not FDA licenced for use in glaucoma surgery.
